Do you use some service api to tell if current user address is spam or how do you prevent spam here?
Also how how do you remove unused old account or should we just leave it or should we clean that periodically??

Misago comes with StopForumSpam integration built-in. It can be enabled from admin panel.
There's feature for deleting user accounts if they were never used to post anything and are older than specified number of days - this is used to prevent forum from gathering personal data of users who don't participate on site.
